I received my RTD last year where my Date of birth states xx-xx-2005. This was the date my Travel document application was recieved by NSC. the put my received date instead of my Birth date. 
I sent that error travel document back to NSC and had to wait almost one year to get back to new one. I had to call almost 15 times to National Customer service. I reapply th form I-131 (no fees). Last time on phone they said me that my travel document was denied. and I have to apply new one with fee. After 2 weeks I received the RTD at my old address. Now I have few qeustions to this forum to insure that I am ok to travel because I'v still found some uncompleted space/section in my TD page.
1/ In Entries section of TD. there is Blank instead of "M" which refer multiple.
2/ In Restrictions section of TD: there is also blank instead of None.
Now, could someone please tell me, that Blank section of Entries would be ok to accept me on port of Entry. In my error TD, there was "M" instead of Blank.
