i got a case number along with pkt3, do i need to fill up and sen consulate any docs

Dear CPers:

It is very urgent please advise me losing no time.

I got my case number assigned last week from nwd it also enloses pkt3 (which i had already sent) and instructions.

Please advise do i need to fill this out again and send it over for getting an interview date.

Please tell me urgently.

is there any other docs that i need to send before interview ?
how much time should i expect for an interview ?
