I-94 form questions

Thanks in advance for your time.

1. On the Form I-94, item 8, "Country where you live -- Lawful Permanent Residence", what should I fill out, U.S. or my country? (I have lived in the U.S. for several years with current 485 pending.)
2. Item 10 and 11, "City where visa was issued" and "Date issued", I'm on 485 pending and come back with AP, no visa. What should I fill them out?

Thank you for the link. But when I read the instruction of I-94 of item 8 "Where do you live" followed by "Lawful Permenant residence", if we fill in "USA", that means we are "Lawful Permenant residence". But I think we are AOS (485 pending), not yet "Lawful Permenant residence". Any thoughts?

I entered US 9 times with advanced parole. Every time I wrote "USA". No problem from CBP officers.
