I-94 and Advance Parole

I'm departing the U.S to Canada and I'll be back after five days on Advance Parole I-131
my question do I have to fill out I-94 when I came back on Advance Parole? I entered the U.S 24 years ago on student visa and now my I-485 is pending.

On form I-94 question # 8 ask city where visa issued and date issued what my answer should be?
