I-90 SFO or San Jose?

I have not yet received my plastic card after
CP and entry via LAX on Dec 13/2000. (That is
15 months). My wife got it on Oct 3rd.

So I am submitting an I-90.
Which office should I use, San jose or SFO?
What should be the specification of the photographs,
that I have to submit?
Would they have transferred my case to the storage,
already?

Earlier, on Nov 20, 2001, I went and restamped my passport,
whereupon an immigration officer, commended that
they have already sent my GC to me and just wait
for three more months before filing I-90.
However I have not received it till now.

city of residence

I have two more questions.
1)I-90 has a line, asking for the city of residence
where you applied for an Immigrant Visa or Adjustment of
Status. (Thy also have another line asking for the consulate
where the visa was granted)

My reasoning is that, since at the time of applying
for CP at chennai Consulate,
I was living in South Bay,california,
my city of residence should be my city of residence in south bay.

2) Along with the I-90 do I have to submit,
the signature and fingerprint blocks of Form I-89?
The instructions in I-90 form says so.
