I-90 Questions - Middle name and First name swapped

Hi,

My middle name and my first name have been swapped in the
cards i received from INS.

I am planning to go to file I-90 with the local INS office. Have
couple of questoins on I-90 :

1. In part 2, question 1, the mention three choices for the status : perm resident(not a commuter), perm resident(commuter), conditional perm res.
Which one should i tick mark ? I dont understand what they mean by commuter versus non commuter.

2. In part 3, what should i fill in for "city of residence where you applied for an immigrant visa or AOS" ?
I have done my Consular Processing in Chennai, and my permanent address
in India is Hyderabad. Which city should I fill in, or should i fill
in city of residence in US ?
