I-90 never received

I got a notice from USCIS stating that I needed to file an I-90 becuase of my card had been returned. I am completing that form now but a couple questions are confusing me. I am on an L1B visa.

1) Consulate where Immigrant Visa was issued or USCIS office where status was Adjusted
Is this the service center I filed my applications, or the port of entry I got my work visa?

2) Date of Admission as an immigrant or Adjustment of Status
Do I put the date I got my visa? Adjustment of status is I-485 I believe, but my I-485 status is still "Case received and pending" for the last year.
 
Apparently you moved while your case was pending, and didn't update USCIS with your new address.

To answer your questions:

1. If you filed I-485 to adjust status, list the USCIS service center where your I-485 was processed. Otherwise, you did consular processing so state the overseas US consulate where you did the permanent resident interview.

2. If you filed I-485, this is the date your I-485 was approved. If you did consular processing, this is the date you first entered the US as a permanent resident.

Before you file the I-90, make an Infopass appointment and go there to find out if your green card was really approved, when it was approved, and what happened to your card and if anything can be done to get it without having to go through the time-consuming I-90 process. Also, if you were approved you can ask them to give an I-551 stamp in your passport (although they may refuse to do this if you haven't filed the I-90).
