I-765 question

Hello,
If I was on OPT status previously (approx 5-6 years ago) and was followed with H1B visa status (still current), what should I fill out when the I-765 question# 11 is asking for which USCIS office was EAD filed and the date?
(I'm not sure if it is referring to OPT during F1 status or H1B or both?)

Question 11 refers to any EAD you obtained in the past. In your case it will be the one obtained through OPT.

Hello again,

I was able to find out through my university what USCIS office the EAD was requested from, but I'm confused which "date" to put.

Here are two "dates" I have in regards to my first OPT.
· Received date: 10/08/2002
· Notice date: 10/15/2002
It was valid from Jan 2003 to Jan 2004

I put the "valid" date (Jan 2003 in your case) on mine.

P.S. Don't be worry about that all that much. I think CIS really care only if you have CURRENT EAD (so they'd issue it as long as it expires).
