I-765 EAD denied without a letter?

Was your wife illegible for EAD to start with?
You said you were waiting for an I-94 replacement when she filed I-765, how about I-485? Was it pending at that moment, or filled concurrently?
Did you moved last fall, by the chance?
Undelivered mail, or ineligibility are only reasons I know people were denied EAD before.
INS should have provided you with a letter, explaining the reasons for denial.
You cannot appeal, but you can reapply. Fill out the I-765 again, pay the fees and don’t forget to include a copy of your wife’s I-485 receipt with the application.

we never moved. neither we nor our lawyer received a letter explaining the denial. we never received anything.

therefore nobody has been able to tell us why she was denied.

and as far as I know there's no reason why my wife should be ineligible.

We filed for the I-94 replacement at the same time that we filed I-485 and I-765, so it was pending for both applications. Now that we have it, how can we "attach" it to our file? is that possible?
 
What was your wife’s status when she entered US?
Did you file her old I-94 with her I-485? If yes, then don’t bother with the update unless INS sends you a specific request for it.
INS wants to see if the applicant entered states legally and in what status, so your wife’s old I-94 should do just fine.
But make sure you bring her new departure/arrival record to the interview.

If you didn’t file her old I-94 with her initial AOS package then you can resubmit a copy now, with a letter referencing her file number.

Did you talk to your lawyer abut all of this?
