I-751 and documents ...several ???

Hi guys,
To those who filed I-751, We are getting ready to file I-751 . In general the whole process seems to be pretty clear. But I still have some minor questions that I can't find answers to.

1> Does the conditional resident submit 2 passport pictures??
2.The Affidavits from friends - do we have to have them notarized?
3. Do we have to enclose Biometrics fee as well, or just the fee of $205 for I-751?
4. Do we have to enclose a copy of every single bill for every single month up to date, or just a few ??? That's a lot of copies.
5. Date of filling- wanted to make sure we file during the 90 day period and not before it starts. I read somewhere that they will return the package if they receive it before the 90-day period.
6. Finally the most stupid question- when you assembled your package - it says Use ACCO paper clips and 2-hole puncturing the top of the paper is appreciated? Is it so - or these assembly instructions are outdated.

I would appreciate any answer. I know the questions are kinda stupid, but we don't want to miss minor details, that can screw up everything. Which is always possible when it comes to burocracy. Thank you in advance!

As far as I know..

1/ Yes - needed for your 10 years green card
2/ Yes - if not, who knows that's really from your friends?
3/ Not sure - Thought I-751 has only 1 fee
4/ I guess 3 lastest one is good
5/ No, they won't return. They will hold it until it's time. We jointly submitted I-751 early and it was on hold until my green card expires. But it's being canceled now because I'm going thru a divorce.
6/ Do not use paper clip or any kind of metal in the mail, it will be denied due to security/terristm. Just put papers in orders and they know what to look for.
