I'm in the process of removing conditions on residence (I-751). Unfortunately, I failed both sets of fingerprints. A few days ago, I received a notice from USCIS saying they need my own signed affidavit regarding any criminal history, supported by police clearance.
I've already gotten a no criminal history letter from my local police department. Now for the affidavit, is it just a simple sentence stating I don't have any criminal history? Also, I don't think it needs to be notarized, is it true? It will be great if anyone can give me a sample of the affidavit.
