I-693 (Please provide latest copy)

I was trying to download the I-693 from USCIS which expired on 06/30/2007. I read on one of the threads that there is new one available in civil surgeons office but not on www.uscis.gov.

I would appreciate if someone can upload the latest version of I-693.

I also got my medical done last week. They used the old forms. The doctor said the forms would be valid. Also, he mentioned that the USCIS has NOT yet uploaded the new forms.
