I-693 Medical Examination

Can you please share some information about I-693. How do I contact the appropriate doctors for this? How long does it take to get this done?

I am in the same boat. Do we pre-fill the form for civil surgeons (Bio part) or they do the whole process and use their own forms?
 
I did the medical examination. I first went to this link

and searched for a civil surgeon in my zip code area. I printed the following form

but apparently the surgeon did not use it as he had the same form prepared with his name and address stamped on it.

The surgeon gave me several vaccinations, and did a checkup, blood sample, etc... After two days, I came back to him and received my report. He gave me a photocopy of my record, and at the same time he gave me the same report enclosed in a stamped envelope that I sent to the USCIS.
 
Thanks JetIst. One more question - Is there a fixed cost of getting the medical exam done or does it vary with which Civil surgeon you go to? Do you know how much does it cost?

It all depends on whether you can prove to the surgeon that you do not need to take all the requested vaccinations because simply you had them back home. That is if you can request your vaccination record from backhome, then you may save yourself about 100$.


In my case, the fees I paid were as follows:
-I recall I paid 85$ for the general checkup and blood test for HIV etc.
-I paid an additional 100$ to do the required vaccinations. I was not able to obtain my vaccination record from back home, otherwise I would have saved myself these 100 bucks!
