It probably depends on the doctor, I asked for a copy and they told, no, we do not give copies... only got sealed envelopes which were mailed to USCIC.
later had problem because doctor forgot to check a box in the form, so went back and got again sealed envelopes with corrected forms - this time, they gave me also a copy of the form but not all records.
