I-693 and I-693 Supplemental were put in one single envelop by my surgeon, OK or not?

The RFI yellow page states:"When appropriately completed, the Form I-693 and the supplement must be sealed in a separate envelope by the civil surgeon. The outside envelope must bear the clinic/doctor's stamp or signature,....."

I don't quite understand the sentence. Pardon my English grammar:

Does this mean the Form I-693 must be put in one envelop, and the Supplemental Form to I-693 in another envelop? Or they can be put together into one envelop?

Everything should be in one envelope..You are ok.
 
Sadasylee..looking at your timeline explains why you are the sad asylee.

No one deserves a GC faster than you my friend...It will be over in less than 6 months..
 
All documents requested by RFI. Is "Proof of Birth Date" needed?
