There's a section in the I-601 Waiver that says:
Part1. Information about applicant (continued)
Applicant was previously in the United States as follows:
and it shows 4 instances where you can fill out US addresses.
Now, my wife thinks you should fill that in for every time you've visited the US but I think it's for people who maybe were students once and had LIVED in the US under a limited visa. There's a box in that section which says 'immigration status' so that would make sense.
I visited the US from Canada about 10 times in 2008, for maybe three days at a time, so it would seem very silly to write all that.
