I-485and G-325A Questions

I am a US citizen. My wife is a Mexican citizen. I am currently preparing I-485 and G-325A for my wife and have a few questions to those who are familiar with I-485 and G-325A.

I-485
1. Date of Last Arrival
I think I asked this question in this forum before but want to ensure this. I don't really know which date should be filled into I-485.

- On I-94, the entry date is May 30, 2010.
- The last entry from USA-Mexico border is July 11, 2010

It is my understanding that the entry via the USA-Mexico border does not give a stamp on her passport. In fact, she has a B-2 border crossing card and does not have to show her passport to the border agent. The border agent does not swipe the B-2 border crossing card to the system. I assume that the entry date indicates the date found in I-94.

2. Application Type
We are pursuing the concurrent processing for both I-130 and I-485 but are confused which selection below should be checked.

a. An immigrant petition giving me an immediately available immigrant visa number that has been approved.

b. My spouse or parent applied for adjustment of status or was granted lawful permanent residence in an immigrant visa category that allows derivative status for spouse and children.

h. Other basis of eligibility.

3. Nonimmigrant Visa Number
My wife does not have a visa on her passport. Instead, she has a B-2 border crossing card and uses this card to enter US. There are 3 lines at the bottom. But I cannot figure out where the non-immigrant visa number is located on her B-2 border crossing card.

4. Father's Name
My wife's father name is not located in her birth certificate. Should I fill out her father's name?

G-325A
1. Applicant
I know that both my wife and I must submit G-325A each. In this form, term "Applicant" is used everywhere. What does this mean? Does "Applicant" mean a person who fills this out? Or does "Applicant" mean the beneficiary (in this case, my wife)?

2. A Number
I naturalized to become a US citizen and had my A number when I was a permanent resident. Do I need to fill my A number?

3. Applicant's last address outside the United States of more than 1 year
So this question is related to #1. If "Applicant" indicates a person who fills and files for (myself), do I need to list my last address outside the US of more than 1 year? I lived overseas more than 20 years ago and don't remember the address.

1. Last entry is the last entry. Write the actual last entry date, not the I-94 date.

2. (a)

3. leave it blank. But make sure to send a copy of the border card with the application.

4. Her father has a name. Write his name, whether or not his name is not on her birth certificate. The question did not say "father's name as displayed on the immigrant's birth certificate".

G-325A: "Applicant" means you on your G-325A, her on her G-325A. Write the A# you had as a permanent resident. Just put the city and country for your last address outside the US ... with you being a citizen and that place being 20 years ago, the exact address is not important.

Thank you once again for your comments. I filled out the last entry date in all the entry date questions except the arrival date regarding I-94 that is located #14 (Arrival/Departure Record I-94, Date arrived) on I-130. I put the I-94 number and the date stamped on I-94. If this date also needs to be the actual last entry date, I will correct this as well.

You mean question 14 on the I-130? OK, I see where your confusion is. It does sort of make it seem like they could be referring to the I-94 issue date. But put the actual arrival date anyway. They know how it works at the border, where people keep the same I-94 while going in and out of Mexico or Canada, so it's not going to cause a problem that the last entry date is different from the I-94 date.
