I-485 Questions

My I-140 petition was approved under EB-2 National Interest Waiver category and I am filing I-485 on my own (without any attorney) at VSC. I would appreciate if somebody guides me through the followings:

1. Part 3 of I-485 states "were you inspected by a US immigration officer?" Does this "inspection" mean the officer who issued I-94 upon arrival?

2. Do I need to file separate I-485 along with mine for my wife and daughter?

3. Where can I find list of INS approved Doctors in New York/Westchester county area?

Yes..
2. Do I need to file separate I-485 along with mine for my wife and daughter?
Yes each applicant is different. You have to attach affdivit of support for both of your dependents.
3. Where can I find list of INS approved Doctors in New York/Westchester county area?
I am not sure.
