I-485 questions. Pleasssssssse respond

Dear Gurus and Attorney's.

I just got my I-140 approved, but cant file 485 because of the backlog. I have couple of questions on I-485 and would appreciate response from likeminded folks

1) I dont have birth certificates of either me nor my wife. I know that I need to get a letter from the local municipal office and affidavit from parents.
Question is that do I need to get affidavit notoraized and signed on a stamp paper or a normal paper. If it is to be done on a stamped paper, do we need to do it on a 20 Rs. stamp of what should be the value of the stamp paper

2) Have any one of you folks had this problem where you did not have the vaccine or immunization records for your or your spose.?? If so, what did you guys do ?? Get new shots ?? Any recommendations

I did not have the record and no hope of getting it. On medical appt they asked about it, I explained the situation, and asked which immunizations were required. After that they hold my results for a day while I went to Health Dept and got all required immunizations and a form with immunization records. I suspect all these immunizations were actual repeats for me - well, my immune system got a second chance :D . I completed them in one day (3 or 4 shots around 80-100$ each), got the form, brought it to medicals office, they attached it to my records and sent me that in sealed envelope.

I had most of the shots taken some times or another but had no records to prove them, so I took all the necessary shots anyway in order to complete my medical papers. It doesn't hurt (but pintch) to take the shots again.

Important fact is, it's a necessary step.

The list of needed immunizations is on internet (I really do not remember where, search for it). We had some records from USA and some from home country, with self-translation. We also did tetanus and MMR in USA, since we knew they are needed, we just went to our doctor and requested them, they were covered by our insurance but it took some time to get them, like doctor should order vaccine...
When we went for green card medical exam we showed what we had, they accepted USA records but did not accept some home country records. Me and spouse had hepatitus vaccinations made in USA, and daughter had it made in home country, and they accepted hers, too (maybe because it had a school stamp on it). They did a blood test, to check antibodies for those additional 'home country' records (I think is was chickenpox), they told if antibody level is ok then no shots needed, if not we will need shots, blood test took 7-10 days (and around $20 per person) and showed we do not need that shots.
