Hi Folks,
I have RFE on I-485 for me and my wife.
My RFE :

My RFE reads as follows.
"Submit affidavit signed by your mother and father,asserting to the facts of your birth and parentage."
What I submitted with I-485:
============================
I submitted my birth certificate taken in year 2003 , though I born in 1969.
My Wife RFE :
=============
My wife RFE reads as follows.
"Submit another Supplimental to Form I-693 , which indicates thet all required vaccinations have been administered,
with appropriate information.The document submitted is unacceptable,because it states that all of the vaccination(except TD) are not required , which appears incorrect."
What my wife submitted with I-485:

The envelope given by the Physician who did my test.
MY QUESTION
===========
1) Any one has any idea about this I-693 form ? . I don't have any idea and my lawer is not accessible (May be on vacation).
Can some suggest me what to do regarding I-693 supplimental form.
2) I got a format from one of my friend for "Birth certificate Affidavit".Can any one give me more links to this format.
