I-485 Portability (AC21) Suggestion needed

The Labor Certification approval reflects the job title of IT Project Manager. I have copy of only ETA-750B.
I can't get a copy of ETA-750A.

Will there be any risk if AC21 is filled without knowing the details of ETA-750A.

I was out of status for 30 Days
prior to filing I-485. Will it affect my approval on using AC21 with change of employer Vs Sticking with the same employer?

Case Detail:
I-140 Approved on Aug 2005 used Sub labour
I-485 RD April 2005.
