Situation:
I just became a USC, wife is on H1-b (came to US on a visit visa). We live apart (I in CA, she in NY) as she is doing her medical residency (still has 1.5 years to go, then she will move back to CA with me).

On form I-485 here are the questions where I'm confused:

"I'm applying for an adjustment to permanent resident status because:

You need to remind us the options pls, nobody memorizes them. It's easier that way.

Your wife will fill out this form since she is one going to adjust her status. She will need to choose option number a which states in its relevant part that a relative visa petition filed with this application that will give you an immediately available visa number, if approved.

If yes, what does it mean by "Attach a copy of the approval notice"?

Don't worry about the wording-"Attach a copy of the approval notice". Didn't you read/comprehend my earlier posting wherein I said what is relevant part of that section? You need to read the whole wording of that section a which states in its relevant part about your situation. Let me highlight that part that applicable to your wife.
