Each company I believe has their own format, and I don't think USCIS really cares about the format as long as it contains crucial information. Mine goes something like:

To USCIS
Mr. So-and-So (Social Security Number xxx-xx-xxx) has been employed by So-and-So company as So-and-so (title) since so-and-so date. Mr. So-and-so is an exempt employee with annual salary of $so-and-so. Mr. So-and-so has his spouse Mrs. So-and-so as his dependent.
Should there be further information required or any questions regarding this letter, contact the Human Service Center bla..bla..bla...

And then I would attach my W2 and pay stub for 3 or 6 months (I forgot)
