I-485 & I-765 Questions - Pls Help!

Hi all,
I am so confused with the questions about last arrival to US on forms I-485 & I-765 - please advise if I should put the day I entered US from my last vacation in Dominican Republic or the first time I entered as an H1B employee 4 years ago??
Also, what I-94 shall I put? The most recent one that came as part of my H1b visa transfer in July 08? I have 2 more the one that they gave me when I came back from the Dominican in 2006 and another one when my previous company has extended my 3 year H1B status in March 08?

Also, on G-325A - if I need more space for the jobs in the last 5 years - do I need to attach a separate sheet (5 of them?). Please advise!!

Did you get new I-94 when entering from vacation. If yes - that's your last admission.

The last one. It will not hurt to send copies of all your I-94 with brief explanation.
