I-485 general questions

HI,
few general questions to the I-485:

1. for the medical exam: I was told by the civil surgeon that he has to attach to his I-693 form in addition to a supplement for the vaccinations also the original print-outs from the lab results and CXR. Is that correct? It seems to me that the I-693 form and supplement should be all that should be in that envelope?

2. For the police clearance: on the I-485 form it says that there is a supplement that lists who needs a police clearance: where is that supplement? Do I need to do that as an H1B in the US applying for EB1-EA?

3. Regarding the photos: do they list specific size instructions?

4. Could anyone remind me what an A-number is (alien number)? I guess that is not the same as on the I-94?

Medical exam send the sealed envelope.
You do not need to do the Police clearance. USCIS will do it for you.
Photographs you can get sample on USCIS web site Or Go to a local photo studio tell them green card photo. They will do it for you.

If you have studied and worked on the OPT here in the US you may have the A#. Other wise leave it blank. It is not I-94 number.

Thus, all I need for my EB1-EA application when I file I-140 concurrently with I-485 is:

1. copy of birth certificate
2. copy of passport page with nonimmigrant visa (simple photocopies enough?)
3. photos
4. medical exam (sealed envelop; do the actual lab test results need to be attached?)
5. form G-325A
6. copy of I-94
7. employment letter (since the EB1-EA is not an employment-based petition, do I need to provide such a letter? Or even if it is not really required is it better to simply provide a letter for the proposed employment???)

For the biometric services and police clearances I understand that this will happen after submission of the I-485 forms, right?

I would like to submit the I-140 with Premium Processing, which is a different PO box than which is indicated for the I-140/I-485 concurrent submission PO box.
Is it ok to submit the whole package with the I-907 for Premium Processing with the I-140 and the I-485 to the same PO box that is indicated for PP at the NSC?
The reason for me to submit concurrently is that I believe that the I-485 my go faster if submitted concurrently with the I-140 via PP.
Would you agree???
