I just went through the process of filling out this form and all the other forms associated with it. When i started, I wish I had a concise list to help figure out what i needed. So in order to help you guys out, I've put this list together as I went along.

As part of this process my wife and I have made a check list for people applying "Change of status based on family".
There are some caveats:
This applies to you if one of the spouse is a US citizen and the other is immigrating into the US. Also, this list is designed for those who will be filling all the applications simultaneously. If you are not filling them at the same time, the mailing address and the costs may differ. Please confirm all requirements before submitting the applications as they may have changed since I posted this.

* Marriage certificate
* Passport (if you have obtained a nonimmigrant visa from a U.S. Embassy or Consulate abroad within the last year, submit a photocopy of the pages of your passport containing the visa(s). )
* I-94 front and back
* If in F-1 status, all SEVIS I-20 or pre-SEVIS I-120 forms
* 2 Passport-style photos (Using pencil or felt pen, lightly print name and A-number on the back of the photo)
* Medical examination in sealed envelope
* G-325A – Biographic Information Sheet (immigrant only)
* I-864 - Affidavit of Support
Fee: $1,010 ($930 plus a biometrics fee of $80, Make Check or Money order payable to U.S. Department of Homeland Security)

* A copy of any document issued to you by USCIS showing your present status in the US, i.e. F-1 visa
* A letter addressed to US Citizenship and Immigration Services stating reason(s) to travel outside the US while the application is pending in the US.
* 2 Passport-style photos (Using pencil or felt pen, lightly print name and A-number on the back of the photo)
Fee: $0 is required to file if you filed I-485
Mail to: Same as I-485

I-765 (assemble in this order)
* Filling Fee
* I-94 front and back

* Birth certificate
* 2 Passport-style photos (Using pencil or felt pen, lightly print name and A-number on the back of the photo)
Fee: $340 (Check or Money order payable to U.S. Department of Homeland Security)

* US naturalization certificate or permanent resident card (if you were born here, you may not need this, passport and birth certificate may suffice.)
* US passport
* Birth certificate
* Marriage certificate
* 1 passport size photo OF Sponsor & Immigrant (Using pencil or felt pen, lightly print name and A-number on the back of the photo)
* Social security number
* G-325A (for both Sponsor & Immigrant)
* Documentation showing co-mingling of financial
* Affidavits sworn to or affirmed by third parties having personal knowledge of the bonafides of the marital relationship.
Fee: $355 (Check or Money order payable to U.S. Department of Homeland Security)
