I 485 filing and lost I 20

Hello
I came to US in 1998 on F1 visa and left in 1999. I have lost the I 20 and do not have the copy of the I 20 or I 94. My university does not have this old document. After i left USA i went to Japan Univeristy and stayed there for 5 years.

In 2004 i came to USA on F1 and moved to H1B. Currently i'm on H1b and have to file I-485. I have all the documents from 2004.

Question -
1. Do i have to disclose the 1998 I20 record in the I485 filing? i:e do i have to submit I 102 along with I 485 application?
2. Is there a way to get a copy of the old I 20 and I 94?
3. Are there any substitute documents for old I 20 and I 94 like getting University transcripts or letter stating that they don't have the copy.

Please help with these issues

You don't need the old documents from 1998. You're not even supposed to keep the I-94 and I-20 after your studies have ended and you've left the US.

You need to provide documentary evidence of your latest legal entry to the US (2004?) -- the I-94 from that entry, and the visa you used for that entry.

Thats exactly what i don't have - a copy of my I 94 from 1998. What other evidence can i show? Can i submit official letter from my US University stating that i completed the course and that they don't have a copy of the original I 20.

I can also have entry stamps to Japan in my passport.

Will this be enough?

I already said that you DO NOT need to provide any evidence of your status in 1998/1999. Your entry stamp from Japan is irrelevant, and your I-20 and I-94 from your studies in 1998/1999 are also irrelevant, and you're not supposed to still have him.

What you need to provide is evidence for your most recent legal entry to the US, which would be 2004 (or sometime after 2004 if you took trips outside the US after your entry in 2004). Provide a copy of the I-94 you obtained the last time you entered the US, and the visa you used at that time.
