I-485 Denied! where & how to file MTR?

Today I received a letter from USCIS, stating that my case has been denied, as I did not show up for fingerprinting.
However I never received such a letter.

So reading on this forum I now need to file a Motion To Reopen!

My question is, does anyone have any Idea how this letter should look like and what to include?

Also, I filed my I-485 with Boston, but the case was transferred to Chicago, as Boston does not process I485 anymore.
Where should I file this? it does not say so in the letter?

My Attorney did not use any form (like I-290) He filed a letter basically saying that it is USCIS error with attached proof of finger print
stamping. There is no filing fee added to it.

My wife dose not have any other status other then EAD, We are filing new 485 under EB2 before September 30 2005, so she can new EAD and AP

Attorney said that we can withdraw new 485 if MTR is opened any approved old 485
