My I-485 was approved on March 7th 2001.
Details:
   India
   EB-3
   Prioriy Date: March 21 1997
   Receipt Date: August 1999
   Notice Date: August 1999
   Finger Printing done: August 2000
   RFE Notice Issued (Employment Letter): January 21st 2001
   RFE Notice Sent: February 21st 2001
   RFE Notice Received: February 27th 2001
   I-485 Approved: March 7th 2001
   Passport Stamped and Finger Prints taken: March 14th 2001
          Took Passport, I-94, I-485 Approval Courtesy Copy, 2 EADs
               2 APs (all these for both me and my wife)
          At Newark INS Office in NJ.

No, my company did not go thru a name change.
I did not check the AVM to see if INS issued me an RFE. I did check the AVM to see if they received it once I sent it to them. the AVM did get updated once they received the RFE.
