1. My case was mixed up with new cases
2. Started faxing twice a week since last one month
3. Luckily one day my lawyer got a very helpful IIO over the phone. IIO promised that she will take personal interest on this case (because it was very old) and make sure that the file is out of the warehouse in couple of days. She did her job as promised and faxed us back the Supervisor name and fax number. We faxed status request query couple of times directly to the Supervisor. With in a week file was moved up to an examiner. Examiner approved the case without sending any RFE with in three days.

Lessons Learned:
Keep pumping faxes and phone calls to INS if the case is old.
