I 485 Adjustment of Status based on 130 Petition - concurrent filing

Hi,
I am applying for permanent residency( I 485) based on relative petition (I 130), I am filing concurrently and I am currently in the US. I have a dual citizenship - Canadian and Slovakian. Should I state both citizenships on the forms? Or is it ok to just state one? (Canadian).

Also, when I came here in October 2009, I came on B2 visitor visa, at that time I took a three months leave of absence from work in Toronto in order to spend time with my (at that time) boyfriend as we had a long distance relationship since April and we wanted to see how the relationship would work. Everything was better than we expected, in December he proposed, we got married in January and I resigned from my job.

On the form it is asking me to state my non immigrant visa number. I don't have any as I only got "B2" in my Canadian passport when I came in October. But then there is a question "When was this visa issued?"

Should I put N/A as this refers to "non-immigrant visa number" which I don't have, or should I put the date I crossed the border and got my passport stamped?

Also, should I attach a cover letter explaining the situation - I want to be able to prove that in October I really came as a visitor. We never lived together before and were in love, but needed to see how things would work in terrestrial life.

(1) Yes you will have to state you citizenship where it required.Both Canadian and Slovakian.i think the form state other citizenship,so you state all.

(2) You came in legally and you where interview by a border officer,so you can adjust your statu9s.

(3)The non immigration visa number is the number on your visa the number on the visa page,B2 visa is a non immigration visa.
Adding i cover letter might not do any good to me for now but you will have to explain during your interview to the immigration office that your intent
was not to migrate to the USA with a non immigration visa,lastly i hope you got married when your I-94 was valid. i have the same case with you and i have already filled my application.

THank you for your advice.

There is no number - it's just a square stamp and it says "B2 Visitor" on my passport page. Nothing else.
I was interviewed by an officer (the usual questions..."Is it for pleasure or business? ") but not "inspected".

On the form it asks: Were you inspected by a US officer?

Should I say "yes?"

I don't have a I 94... I didn't need one, as a Canadian...
But yes, I am allowed to stay until April... (six months for Canadians with no work or any other privilegdes)

you can change status,just put N/A and make photocopy of you passport and visa page along with the document you are sending.
the visa i think people cant fill they are under visa waiver programed.
