I-140 Premium processing

How long after LC approval did you file for I-140? I am in a similar situation. Just got LC approval a few days ago. Was thinking of filing I-140 premium processing, so then I can apply for my 3 yr extension once the approval comes.
Question was:
1) How long does I-140 premium processing take?
2) Once the I-140 is done, then do I first switch H1-B to the new company and then once the 6 month period before current H1B expiry approaches, I can then apply for the 3 yr extension.
The other question was how easy is it to change jobs after the I-140 is approved. Does one have to restart the whole process or can one use the approved I-140 and file the I-485 with the new employer. Other problem is PD is not current due to retrogression.
