1. Should the cover letter before or after the I-140 form?

2. Should I put a list of materials on the top?

what happened to you eb1 or application? are u reapplying under the same category?
the cover letter should go before I-140 form.

Sometimes people opt to include the list of materials in the cover letter. In which case you'd have
- check
- cover letter
- I-140 form
- supporting materials

There's also a helpful site about material assembly, http://www.uscis.gov/graphics/fieldoffices/scnational/index.htm.

I binded the materials like

- check (money talks, right ;) )
- cover letter
- form I-140
- Supporting stuff


I think now the package already arrived at the center :)

Do we need to provide photocopies of passport, visa, I-20, I-797, I-94 etc with the I-140 form? If so, do these need to be notarized? How about the copies of diploma etc?
 
I attached copies of passport and H1B1 approval to prove I am pplu ;)
