I-140 approved, got promotion, need extention now

I got my i-140 approved in July, My H-1 expires in January. I got promotion( substantially different responsibilities) in September. I need to get extension , I filed a new labor certification but By mistake I said I would get 3 years extension! Darn

1- do I need to file a new PERM?
2- how about the I-140?
3- i HAVE A VERY SHORT TIME , WHAT SHALL i DO?
