I-140 amendment Questions

Friends,

Due to my company name change I have to file a new I-140 amendment.
I have follwoing questions:

1. Is there any column on I-140 where we need to mention the reference
   to old I-140?
2. What is list of documents to be submitted for I-140 amendment
   application?

Did your lawyer adviced you to file Am. I 140?
Let me explain my case and then you decide whether to file or not.
My case was current 3-4 months ago and was waiting for the company name change hold. Lawyer asked me to file Am. I 140, and I have to pay for it since it is a proactive one( normal GC expenses are taken care of by the company). And I did on May 23rd, since there was no regulation at that time. On June 19th, they released a new regulation, so you dont need an am. I 140 any more.(I know cases where people got approvals without A,I 140 and cases like mine where I am still waiting)
Check with your lawyer about AC21, especially if you are paying.
Regarding the application, you have to mention that it is only an amended application and a normal I 140 has been approved, please expedite. Also there is a column to put your A#

I think you need a new I 140 since the AC21 talks about cases where company name changed(or you changed job) after 485 application I guess.
Still you should be able to expedite the case.
