I-134 affidavit of support

Hi Friends,
I have to fill I-134 form for my spouse and 8 year old kid.

I am wondering whether it is enough to use one I-134 form for both of them or do I need to use one form for each of them?

Why do you have to do affidavit of support. Isn't it assumed that a non-working spouse and children are by default supported by the primary bread earner?
