I-134 (Affidavit of Support)

It is to be presented at the time of interview in the consulate.

It is for however many dependents you have.

I also read that it shd be notarised. Is it true.
 
You personally by the virtue of your Employment based IV, are showing the ability to support yourself and hence would not be a burden on the state. So one does not need for oneself.

Your spouse (and other dependents like children) are not the direct benificiary of employment based IV and hence how they will sustain without the burden on the state? By you swearing that you will support them, by the virtue of I-134, the issue does not arise.

Normally, most of the people I have heard have notarized the I-134 form. But I have been made to understand that you can choose to sign it in front of the interview US consul officer (which is logical).

So if you sign it, it has to be notarized. If you do not notarize it do not sign it beforehand.

As an option you can take both a signed notarized form and a non-signed non-notarized form.

I doubt if anyone would actually send you a filled copy of
I-134 since it contains some confidential information.

Most of the columns are self-explanatory, however if you need
help with specific items, just post your query and someone would
respond.

By the way you can find this form on the net. Just search by
keyword 'Form I-134' or 'Affidavit of Support'.

In Question 2. I don't qualify as any of the options:-

If you are not a native born United States citizen, answer the following as appropriate:
a. If a United States citizen through naturalization, give certificate of naturalization number
b. If a United States citizen through parent(s) or marriage, give citizenship certificate number
c. If United States citizenship was derived by some other method, attach a statement of explanation.
d. If a lawfully admitted permanent resident of the United States, give "A" number

All are N/A for me. So will I be qualified to Support my Spouse.

What shd I answer here, assuming we do not have children.

{You can write 'None' in this item. This is one item which
poses some confusion to the candidates. Possibly this item
would make sense if you were preparing one single I-134
for several people. But that is not the case now. Each
candidate (dependent) is required to carry a separate I-134.}

Here is my take on Q3

Spouse name and details go in the main boxes. It then asks for spouse and children accompanying the "person" i.e. your spouse. So in case of no children, 'none' or '-' should work. I put '-' in all those boxes as I was filling I134 for spouse only.

If this is for parents then usually you have to fill one for each. in that case

1) When filling out the form for father, mother's name goes in the box for spouse
2) AND when filling out the form for mother, father's name goes in the box for spouse

Here again I am writing from my own experience.

Raju is trying to point you to the closest option, as U are defintely not a US citizen (1st 3) and you have to pick one of them.

2nd Q - I don't know. But if I were you, I wound not try to unnecessary complicate things by sponsering parents at the time of interview. It could always be done a few weeks later.

Getting sample fo I-134 is definitely out-of-question due to confidential information. AS suggested by raju, enter h-1b visa approval number againsst line (d) and state h-1b visa in bold.
Dont get stressed out over things not worth getting stressed over.

Sorry to have created the perception that I sponsored my parents during CP.

No I did not.

I filed I-134 for my parents when they visited me on their B2 (tourist) visa. The point I was making was that the spouse information in Q3 is regarding the person being sponsored.
