I-131 application

Please help me about the I-131 AP applcation.
What is the correct address to send I-131 advance parole to CSC center? I have EB3 I-485 pending there? Can I just send to Po box 30111 or there is another PO box number?
What kind of support document I need to send besides I-485 notice, EAD card?
