Filling out the form right now and everything looks pretty straightforward, however, I have a question about
"21. If filing for your husband/wife, give last address at which you lived together. (Include street, city, province, if any, and country)" - shall I give the current address since we are living together in the US, or the one we had before?

The address you live now. That's what I did, anyway.
