I-130 / G-325A question

Hi,

I have my packet ready and close to sending it to Chicago today, except I need these 2 things. I searched the forums and I got a partial answer to this.

With the I-130 form I have 2 copies of G-325A forms.
One I filled completely, signed it myself and in the heavy bordered box at the bottom where it says Applicant: I filled in my Family Name/Given Name and Middle Name.
The other copy of G-325A form, my wife filled it completely and signed it.
My question is, should she write her Family Name/Given Name and Middle Name in the heavy bordered box at the bottom where it says Applicant, or write my name ?

Secondly, on the I-130 #14. If your relative is in the US, He or She arrived as a: ........ I-94 Number ........ Arrival Date .........

On the above question should I fill in the details about when I first arrived in the US (F1/Jan 99) or my most recent entry into the US (H1-B/June 07)?

(I don't have the details for my I-94 during my first entry (Jan 99). So I won't be able to fill that in. I have currently filled in my most recent entry to the US.)

G325A question - that is her biographic information so her name should be at the bottom.

The other question - I think your last port of entry. ( I assume, when you leave the country they take your I-94, so according to that I-94 you are no longer here. Now they ask you if you are in the country. So your latest I-94 tells them you are here. )

PS - I happen to call the Customer Service at USICS (before I took a look at your reply) and asked them about the G-325A form question and they clarified the same.
