I-130 data question for question B-14

Hello everyone!

My dad plans on filing a I-130 both for my younger brother and me. But there has been some argument as to what exactly needs to be put into the B-14 section of the form:

- 'date, place and class of admission'

He is a Permanent Resident (has also left and re-entered the country as LPR before... in case that matters?!) and used to be here on H1B before he got married (class would thus be IR6 aka spouse, adjustment of status right? or just PR as suggested by others in older postings before?).
He has as "received date" on the "I-485" application: Application to register permanent residency or adjust status the 05/26/2004, which was then granted on 09/14/2004. It is thus the latter to take as date of admission right?
He's living in NYC and I believe he also filed it there, but says he has as place of admission the MISSOURI SERVICE CENTER with P.O. BOX 648005, LEE'S SUMMIT, MISSOURI 64064.

I find this somewhat weird. Also, B-14 clearly doesn't give enough space to put in a whole address so I figure it's either just New York City or Lee's Summit - though this really is just guess work here. Which one is it?

I figured some other LPR must have filled out I-130 at one point in time before and know for sure what to put in there. Or at least where to get these information as it has left me highly confusing at this point :confused:. I have seriously browsed through all immigration forums I could find (including all old postings in this one!!) but couldn't get a clear answer so far. :(

It should be the date and place his I-485 was approved OR the date and place he first arrived in the US on an immigrant visa. Just the name of city is enough, address is not required.

According to http://boards.immigration.com/attachment.php?attachmentid=13995, his class of admission should be IR6 now - so I'll go with this one. And 09/14/2004 as date and Lee's Summit as place.

Would it be relevant if he has removed his conditional status (transfer into unconditional LPR) later on? That is, would he have gotten a new date and place of admission (of the new, unconditional status)? The dates given here were for his I-485, but as far a I know that only made him into a conditional permanent resident (which would thus be class CR6 instead). Or does this not really matter?
