When you fill in Part B it will ask family or individual, if you are applying for more than one person you should select family based.
$275 for each person. Single mailing fee is good enough for a family.

2)it says we have to send "duplicate copy" what does it mean?

Just another copy of the documents.

3)does it necessary to notorised all copy? or self attached?/

I applied in SFO and kept a notarized copy of my son's PIO card. Rest of the forms I just signed as stating that its "TRUE COPY OF ORIGINAL"

4) do i have to make two copy and sign each copy?? why

I think so.

MY answers apply for SFO Indian consulate . It may vary between different consulates, So please also check with where you are applying.

OCI application question

Hello All,
I am trying to apply for OCI and have few questions:

1.) Where do we have to apply? Embassy of India in Washington DC or consulate which covers my area of residence. I am currently residing in New Jersey.

2.) I understand that we have to make two copies of each Part A and Part B. Do we have to send both the copies to the same address?

3.) I am a US citizen. What documents would I need to send for OCI? Do I need to send my original US passport?

I appreciate your help.

Thanks
patkt

Being in NJ you would come under the jurisdiction of the Consulate in NYC which is a good thing (it has the fastest turnaround times for OCI).

2.) I understand that we have to make two copies of each Part A and Part B. Do we have to send both the copies to the same address?

Yes you send them together in the same envelope.

3.) I am a US citizen. What documents would I need to send for OCI? Do I need to send my original US passport?

The US passport will do for now but you shouldn't send the original until you get online confirmation that your OCI has been granted (may take up to a month at the NYC Consulate). For the initial application a notarized copy of the data & signature pages of the passport should do. Do remember to enclose 2 copies of all the supporting documents (you will also need copies of your old Indian passport as proof that you had previously held Indian citizenship).
