how to send birth certificate to INS?

I\'ll do FP next month. When I submitted I-485, I didn\'t attach birth certificate because I didn\'t have it in hand at that time. I think it\'s a good chance to submit birth certificate when I do FP. I know some one talks about "yellow form". Could anyone kindly tell me how to do it?
