How to respond to RFE?

I need your help in replying a RFE.
Do I have to send in the copy of application form again, with RFE letter from INS and requested document? Or I just need to send the RFE notice and the required document only?

Urgent help needed!

Only the documents requested by INS along with Original RFE Notice. Please check with your lawyer also.
