How to qualify my Advance parole request

Hi, all
I am trying to apply for Advance Parole travel document. But it seems, from the instruction, they approve emergent travels sparsingly. It also asks to qualify myself for an issuance of an AP. Can anybody help me or post an example of the separate sheet of paper?

All those statements are just standard stuff. Don't worry about that. Just write down that your I-485 is pending (I hope that is the case) and send in the reqd docs and they will give it. I applied it for my wife and got it last month.
