How to inform USCIS using AC21?

I have decided to inform USCIS about my job change (by virtue of AC21). How do I initiate this activity? What documents should I send? Is there any standard letter that I need to send?

Please help.
 
Here is what my Attorney sent

A covering letter which states the conditions of AC-21 and stating that you meet them. (Condition means just your I-485 is pending for more then six months.)The covering letter included the EAC number and A# of beneficiary well as dependents
As proofs they included the experts of AC-21 law , August 2003 memo (about revoking of I-140) and new employment letter.
