How to get Report of commercial rating concern

Hello,

I will be filing affidavit of support(I-134) for one my relatives. I
am self employed. I have two option for supporting document


1) Copy of last year's Income tax return
2) Report of comm. rating concern

My question is how to get report of comm. rating concern and from
where? How long it takes to get and how much it cost to get.

I know , I can submit my return along with W2 or Pay stub but would
like to see how easy/difficult the 2nd option is.

Any help would be highly appreciated.
