Fill up misc services form. This is available on www.immihelp.com site under fillableforms/India. The fees is $11 (MO or cash). Also take a copy of your current visa / approavl notice (H1, etc).
You can either go to Houston in person or do it by mail (add MO for $15 return by Fedex). They need your original passport as well. If your passport has been reissued in Houston then it takes just a day (give application in the morning, get it by evening) + mailing time if any. Else, the time they are giving is 30-45 days, as they have to contact RPO in India. It just depends on when they reply. I think you can pay an additional $22 to speed this up a little (for them to use telex instead of mail). If yours is the latter case, I heard you just need to send a copy of your passport initially, but they will need the original to issue PCC to you which you can produce later (say once they hear back from the RPO).
