How to get copies of Labor / I140 from INS

Can some one post information about 'Getting copies of labor and I140 copies using FOIA'

Information I need is

1) The letter format for this request

2)Whom to send this letter

3)What supporting documents we should send along with letter

4)How much time it takes to get the copies from INS

Fill up G-639 form and send it to your processing center. No documents need to be attached besides providing info. in G-639 form. As simple as that. Response time is huge atleast around 7-8 or more months.
