How to fill out Part 2, Box 4, of I-907, when an organization is petitioning?

Hello, I am Premium-Processing an H1-B extension for one of our employees. The petitioner is the organization I work for. I am using the latest version of the I-907 and I am having some doubts about Part 2, Box 4: The petitioner is an organization, and in this version of the form you seem to be allowed only to enter a person's name. Should I enter the name of the organization in the family name box and leave the others empty or should I enter the name of the organization's point of contact (which I have entered already in box 6)?
